Thank you for your interest in Westhampton Elementary School! Below you will find information regarding student registration.

Please complete the registration paperwork request on this page or call the office to obtain a registration packet.

If you would like to stop by the school to register your child, please bring the following documentation:

1. Certified copy of birth certificate (long form)

2. Immunization and physical records

3. Proof of residency (three documents):

-Mortgage statement or lease agreement including the parent/guardian’s current physical address -Current utility bill statement at said address -Valid government issued I.D. such as a Massachusetts driver’s license, voter registration card, or state I.D.

4. Previous School’s Records:

-Attendance records -Academic transcript/report cards -Special education records & IEP (if applicable) -Section 504 plan (if applicable)

5. If applicable, shared custody agreement in the event the child being enrolled is in separate custody of two or more parties.
